Franklin County Rehab Center is the newest skilled nursing facility built in Vermont, with 64 licensed beds. Franklin County Rehab Center is locally owned and operated, with a high staff to resident ratio and winner of the State of Vermont’s Gold Star Employer Award for the years 2005-2011.
Franklin County Rehab Center has been rated a five star nursing facility from Medicare. We are also proud to announce that we are a recipient of the 2010, 2011 & 2013 Nursing Home Quality Award given by the State of Vermont. The Nursing Home Quality Award recognizes outstanding nursing homes which have met high standards of quality of care for Vermont’s nursing home residents. This award is granted based on results of health and life safety surveys, cost effectiveness, a lack of substantiated complaints and resident satisfaction survey results.

We offer short-term rehabilitation services, including: physical therapy, occupational therapy and speech therapy. The rehabilitation team creates an individualized treatment program developed with the interdisciplinary team of nursing, social services, activities and dietary in consultation with your physicians. Returning you to your home is the team member’s objective. They will play an integral role in helping you regain your health and reach your maximum ability.
We also provide long-term care services to residents with multiple healthcare needs requiring 24 hour care. This care is delivered through an interdisciplinary approach with our team of licensed and certified medical care providers. We strive to meet the social, recreational and spiritual needs of residents with stimulating activity programs, and a comfortable home-like environment.
